    • See “Rights and Responsibilities” below.

  2. Have the following documents submitted to [email protected] before your appointment:

    • Proof of Identity

    • Proof of Income

      • If your household has Medicaid, SNAP (Foodstamps), or TANF we can use your CIN# or case letter as proof of income and do not need to see other documents for income eligibility. If you do not have Medicaid, SNAP (Foodstamps), or TANF, we need to see the last 30 days of income for your household. 

    • Proof of Residency

    • Proof of Category

      • Proof of category is asked to be provided at your appointment, but if it is not available at this time it can be provided at a future appointment.

    • See “What to Bring to Your Appointment” below for details on each category of proof requested.

Rights and Responsibilities

Applicants should review the Rights and Responsibilities before their appointment or at the certification appointment. This will certify that the information they have provided for their eligibility determination is complete and correct to the best of their knowledge and that this information is being given in order to receive WIC benefits.

Household Changes During a Certification

Per the Rights and Responsibilities, you must notify your WIC local agency if you move, your phone number changes, the income changes for anyone in the household, or if you want to change to another WIC local agency.

You may be asked to provide eligibility information mid-certification if a change occurs. Local agency staff will review what is required as needed.

What is a certification appointment?

There are 2 types of certification appointments:

  • Initial Certification Appointment

    • Initial Certification Appointments are the first type of appointment you will have when enrolling on the WIC program. At these certification appointments staff will assess eligibility and review information about how the WIC program works and how to go shopping.

    • A nutritionist will complete a nutrition and health assessment and review your benefit package.

 

  • Recertification Appointment

    • Recertification Appointments are scheduled when someone has already been on WIC, but is due to assess eligibility again. At these certification appointments, staff will assess eligibility and review additional information as needed.

    • A nutritionist will complete a nutrition and health assessment and review your benefit package.

 

Certification appointments tend to be the longest appointments, however they occur on a fairly regular schedule! WIC staff will review with you when it is time to recertify again.

  • Infant Certifications can last from 6 months to 1 year.

  • Child Certifications last 1 year.

  • Prenatal Certifications last through the pregnancy.

  • Postpartum Certifications last until 6 months after the pregnancy ends.

  • Breastfeeding Postpartum Certifications can last until an infant is 1 year old.
