Tips for a Successful Certification Appointment

  • Appointments can be completed in-person or over the phone, whichever you prefer!

  • For phone appointments, make sure WIC has your current phone number, your phone is on, and your voicemail is set up. We will call twice – if you miss both calls, check your voicemail for a message from our staff.

  • Text us your documents before your appointment. We’ll send you a secure link to upload your documents to – if you don’t receive it or it isn’t working, call us at 315-435-3304.

What to Expect

Certification appointments are required for all new WIC participants, when changing from pregnant to postpartum or breastfeeding, and annually for children.

Part 1: Eligibility Assessment

All information shared with WIC will be kept confidential. WIC Support Staff will review eligibility criteria, ask additional questions required by the New York State WIC Program, and mail/give you a eWIC card if needed.

For in-person appointments, staff will measure your/your child’s height and weight and perform a finger prick iron test.

You will be asked to provide:

  • Proof of Identification
  • Proof of Address
  • Proof of participation in Medicaid, SNAP, TANF, Head Start/Early Head Start, or Essential Plan (levels 1-4, or pregnant and postpartum only: levels 200-250) or proof of household income

Please note that this information must be provided at all certification and recertification appointments. 

You may also be asked to provide:

  • Recent height, weight and/or bloodwork for you/your child
    • Screenshot from Patient Portal or Health Care Provider Visit Summary
    • WIC Medical Referral Form completed by your Health Care Provider
  • Proof of pregnancy
    • Sonogram photo
    • Statement from your Health Care Provider with your due date
  • WIC Medical Documentation Form completed by your Health Care Provider (required if your child is on a special formula)

Part 2: Health & Nutrition Assessment

WIC Nutrition Staff will:

  • Ask health and nutrition questions about you/your child
  • Provide opportunities to discuss nutrition, breastfeeding, and overall wellness
  • Provide breastfeeding/chestfeeding support and refer to a WIC Breastfeeding Peer Counselor if needed
  • Provide referrals to other programs and services
  • Issue benefits to your eWIC card
  • Schedule your next WIC appointment
