Online Certification and Re-certification

Follow this process to certify or re-certify online

1. Read Rights and Responsibilities: English, Español, 中文 (Chinese), বাংলা (Bengali), اَلْعَرَبِيَّةُ (Arabic), हिन्दी (Hindi), नेपाली (Nepali)
2. Please prepare the following documents before contacting us:
   • If you have Medicaid, you need to provide:
     1. Active medicaid numbers for you and/or your child or Self Declaration Form: English, Español, 中文 (Chinese), বাংলা (Bengali) ,اَلْعَرَبِيَّةُ (Arabic), हिन्दी (Hindi), नेपाली. (Nepali)
     2. ID card
     3. Medical referral form or MRN#
   • If you don’t have Medicaid, you need to provide:
     1. Proof of income or Self Declaration Form: English, Español, 中文 (Chinese), বাংলা (Bengali) ,اَلْعَرَبِيَّةُ (Arabic), हिन्दी (Hindi), नेपाली. (Nepali)
     2. Proof of address
     3. ID card
     4. Medical referral form or MRN#
3. If you have all the documents above, please contact your WIC office to apply.
4. WIC staff will do their best to provide benefits for you remotely.
